When downloading the Community Chest Application form, you will need to view the form in a web layout. To do this click on the view toolbar and go to the web layout. Please remember to send your completed application form to the Community Chest Officer in your Local Authority.

Sportsmatch Application Form
PLEASE NOTE: It has been noted that the new Sportsmatch Application Packs no longer ask for a 'Contract of Agreement'. However; it is still a requirement that every sponsor must either;
- Send a letter to the club confirming the agreement (for apps. under £10,000)
OR - Complete a 'Contract of Agreement' (for apps. over £10,000)
Sports Council for Wales will require copies of these agreements, in addition to the Declaration contained in the application pack. Please download an example contract of agreement below. Please also note that application must be received at least 6 weeks before the project is due to start. We also ask that you do not start to spend/invest your Sponsor's contribution, until the assessment of your application has been completed.
