Community Chest Application Form
When downloading the Community Chest Application form, you will need to view the form in a web layout. To do this click on the view toolbar and go to the web layout. Please remember to send your completed application form to the Community Chest Officer in your Local Authority.

Sportsmatch Application Form
PLEASE NOTE THE FOLLOWING IMPORTANT INFORMATION:
- Applications must be received at least 6 weeks before the project is due to start.
- Do not start to spend/invest your Sponsor's contribution, until the Sports Council for Wales has notified you of the outcome of your application.
- Please submit a ‘Contract of Agreement’ with your application. To assist you with this an example ‘Contract of Agreement’ is available for downloading below.
